Please note, this form is email address sensitive, so please use two different emails for your forms, so you don't overwrite each other.

Then, return to this page and book your personal Medicare review with North Shore Senior Healthcare and gain peace of mind knowing you’re in the best plan for you.

After completing your profile, please schedule an appointment with me using this schedule an appointment link.  You will need to agree to a scope of appointment 48 hours before we can meet. Additional attendees (such as your spouse) are required to sign the “Scope of Appointment” form at least 48 hours before our appointment. They will receive a link to complete the form via email and text. Please make sure they follow the link and complete the form. Once done, I will do my due diligence and be ready for our meeting.
